Understand the importance of Advance Care Directives with VIA Health Partners. Learn how to plan your care decisions and ensure your wishes are honored. Advance Care Planning Honoring the wishes of your loved one. Advance care directives provide clarity for when the time comes to use them. An advance directive is a legal document that outlines choices about being kept alive by artificial means when suffering from a terminal illness, permanent brain damage, or advanced dementia. Such documents are utilized when we lose decision-making capacity. While many of us have strong opinions about whether we would like to receive these types of treatments in an attempt to keep us alive, few of us actually take the steps to ensure that our wishes are honored. Studies have found that most of us would refuse to be kept alive by artificial means if we have dementia or a terminal illness*. Studies have also found that only 50% of terminally ill patients execute advance directives**. However, there are several easily completed documents that will allow loved ones to understand a patient's wishes regarding end-of-life care, even if the patient lacks decision-making capacity. The state of North Carolina recognizes two advance directive documents. Revised in 2007, they are the Heath Care Power of Attorney and the Living Will. These documents are legal when they are completed by an individual with decision-making capacity, signed by two witnesses, and notarized. There are also two medical orders recognized by North Carolina. They are the Do Not Resuscitate (DNR) and Medical Orders for Scope of Treatment (MOST) forms. These forms are obtained from the physician or healthcare provider. Health Care Power of Attorney Health Care Power of Attorney The Health Care Power of Attorney allows a person to name someone to make healthcare decisions for them, should the person be unable to communicate their wishes. Advance Directive for a Natural Death (Living Will) Advance Directive for a Natural Death (Living Will) The Living Will should be used as instructions for the future for anyone who would like their healthcare providers to withhold or withdraw life-prolonging measures in certain situations. There is no legal requirement that anyone execute a living will. Do Not Resuscitate (DNR) Do Not Resuscitate (DNR) The DNR form is a medical order that communicates a person’s desire to forgo cardiopulmonary resuscitation should their heart stop beating and they stop breathing. It is utilized outside the hospital setting, and is bright yellow with a red stop sign in the middle. Medical Orders for Scope of Treatment (MOST) Medical Orders for Scope of Treatment (MOST) In North Carolina the MOST form is a medical order that describes a patient's end-of-life plan of care. It is intended for patients experiencing a terminal illness with a prognosis of less than one year. Since it is a medical order, any physician in any setting can follow the orders. It is also bright pink so that it can be easily identified. Physician Orders for Scope of Treatment (POST) Physician Orders for Scope of Treatment (POST) In South Carolina a POST form is a designated document designed for use as part of Advance Care Planning (ACP) and consists of a set of medical orders signed by a patient’s physician addressing key medical decisions consistent with patient goals of care concerning treatment at the end-of-life that is portable and valid across health care settings. A POST form should be completed by a trained facilitator or qualified health care professional in conversation with the patient. Caring for a loved one with dementia is difficult, but you're not alone... Caring for a loved one with dementia is often very difficult, with family and other caregivers experiencing high levels of both physical and emotional stress. In an effort to alleviate some of this anxiety, VIA Health Partners has created a specialized care plan for patients diagnosed with dementia, which also includes support and education for caregivers. The extensive training provides tools to physicians, RNs, nursing assistants, social workers, chaplains, and volunteers and includes instruction on increasing or maintaining a patient’s quality of life, appropriate pain and symptom management, and Advance Care Planning (i.e. Living Will, Health Care Power of Attorney). There is even social support for caregivers. We have trained clinical staff to understand the symptoms of dementia and we have introduced a pain assessment tool specifically designed for patients with impaired communication. These teams also work with therapeutic sensory kits containing aromatherapy scents and tactile stimuli, incorporating items reminiscent of their patients' childhood or career, music from their youth, and more. These kits are designed to stimulate the brain and increase communication with both the care team and family members. Call us at 833.839.1113 for more information or to schedule a consultation. More resources: Caregiver Resources >> Patient & Family Resources >> Making an impact with iPads Making an impact with iPads Sometimes the best medicine is unexpected, as we've found within our iPad program... Our staff have had great success using iPads with specific applications to interest patients with dementia. Social workers use iPads with specific applications that have produced interaction with patients. Over the past several years, we have seen amazing results. Even our most unresponsive patients have interacted with the iPads that have been brought to visits. A diagnosis of dementia can be frightening for a family, but this new program offers a glimpse of the loved one they know is still there. This initiative is being implemented in homes, assisted-living communities, and skilled nursing facilities. We believe that this new program will make a significant impact in the end-of-life care that our patients with advanced dementia receive. In order to offer even better care to our patients living with dementia, VIA Health Partners has a goal of purchasing iPads for all of our social workers to use with this project. You can help us reach this goal by making a gift to fund this meaningful project. Thank you to the Mecklenburg Medical Alliance & Endowment for their grant to support "Positive Approach to Care," a hospice training program that provides volunteers and staff with a deeper understanding of dementia and dementia related behaviors.
