We support. We care... Grief comes at different points along the journey of loss, but the primary points are the period that begins at diagnosis and the period that begins at the death of a loved one. From the point of diagnosis to death, there are two types of grief: preparatory and anticipatory . Preparatory Grief is experienced by the person who is ill as they grieve the loss of their health, independence, and the shortened time they will have with others. Anticipatory Grief is experienced by family and friends as they struggle with thoughts and feelings surrounding the impending loss of their loved one. In the hospice and palliative care settings, these losses are typically processed with social workers and/or chaplains who are specifically trained to deal with these experiences. Once death has occurred, feelings, thoughts, physical reactions, and other aspects of grief can make adjustment difficult, and carrying out basic activities may seem harder. Family, friends, and other people you know and trust may have difficulty sharing in your grief or they may be struggling with the loss themselves. That’s when the grief care services of VIA Health Partners can be so important. We listen. We support. We care. Our staff of professional counselors has a wide range of experience in grief counseling. All hold professional licenses and/or certifications and have degrees covering social work, counseling, and divinity. Counseling for adults, children and teens is available throughout our 49-county service area. Our grief counselors create a safe environment for individuals who are adjusting to a significant loss. If you have survived the loss of a loved one, we can help you discover, or rediscover, inner resources that will promote healing and help you define a meaningful future. Individual counseling. . . Talking one-on-one with a grief counselor helps to make sense of the grief process. The way in which a person grieves is unique to that person. Having a grief counselor to help you work through your emotions and thoughts about your loved one, share your memories, and discuss adjustments to life is healthy; it helps you recognize that what you are going through, though painful, is a natural life experience. We employ a beneficial perspective that is based on the following principles: Pain and loss are a natural part of life and grieving is a healthy way of taking care of ourselves. Grieving persons have the right and responsibility to grieve in their own way, to interpret their loss, to make choices that are right for them, and to determine the quality and direction for their lives. The support group allows group members a safe environment to experience empathetic support and a place where social isolation and feelings of loneliness are diminished. Groups specific to spousal/partner loss, adult parent loss, adult child loss, young widow/widower, mature widower loss, and child loss may also be offered. A pre-group interview with a grief counselor is required for members of structured grief groups. Please contact us for more information. "In Good Company" Monthly Drop-ins If you are interested in a more relaxed, informal group setting, come to one of our "In Good Company" support groups . We offer camaraderie and an opportunity to check in with a grief counselor (and others) to share the highs and lows common to the experience of people who are grieving. We have groups that meet monthly throughout our service area. Please contact us for more information. In School Grief Groups School personnel are encouraged to contact us with requests for in-school grief support groups. In other words, we not only care for the family members of patients under our care, but we will offer our services free of charge to anyone who is struggling with loss. Any individual in our service area may sign up for our grief support groups or workshops, or contact a counselor for additional assistance. Memorial Services Remembering those we love and have passed away is an important part of individual and family identity. Attending a memorial service is healing, connects the present with the past, and continues the bond of a loving relationship. Each year, family survivors and community members receive invitations for our Light Up a Life interfaith memorial services held throughout our 49-county service area. This service may be used for stress management, marital/family issues, financial concerns, alcohol and/or drug abuse, job/career issues, communication skills, parenting, legal issues, personal/emotional concerns, grief and loss, etc. Flexible Spending Accounts To help reduce the health and dental cost to employees, VIA Health Partners sponsors a Section 125 Plan. VIA Health Partners provides hospice care in the home, in long-term care communities, in hospitals, and in our 6 inpatient hospice houses. What is Palliative Medicine? Palliative medicine is health care for individuals of any age who have a chronic, life-limiting illness. Palliative medicine is active care. It is care that aggressively seeks to improve quality of life by focusing on physical symptoms as well as emotional, social, and spiritual needs. While palliative medicine emphasizes pain control and other symptom management, patients do not have to forgo curative or restorative care. Palliative medicine also reaches out to address the needs of family members and caregivers to maximize support to the patient. Palliative medicine is provided by our Palliative Medicine Consultants program. What is the role of nurse on the hospice care team? Our staff nurses provide intermittent skilled nursing visits for pain and symptom management, plus education regarding care of the patient. How are the patient's personal care needs handled? When ordered by a physician, we provide certified, in-home aide services to help patients with personal care, including bathing, skin, mouth, and hair care. What is a "caregiving plan?" When a patient comes under hospice or palliative care, a plan of care is developed based on the needs of the patient and their family. This plan outlines the multilevel, clinical services that will be provided, and it is updated regularly to reflect the changing needs of the patient and family. What is the role of the medical social worker on the hospice care team? Our social workers provide counseling and support to the patient and family, including children, during this stressful time. Services may include assistance in identifying community resources, advice regarding financial, legal, and caregiving concerns that occur with a prolonged illness, as well as other individual services as appropriate. How is spiritual care provided? Our staff chaplains provide and/or coordinate spiritual care for patients and families by helping them find the spiritual comfort, strength, and hope consistent with their own values, beliefs, and practices. Spiritual care can also offer the support needed to explore, clarify, or resolve spiritual issues of concern. What bereavement or grief care services are available and to whom? We recognize the special needs of those who have experienced the death of a loved one, and we provide the individual and group support necessary to face grief. Grief services are available to families and loved ones of our patients, and are also available to anyone who resides in our community. What if the patient needs hospitalization? If hospitalization is needed, the care team will coordinate this transition and maintain contact with the patient and their family during the patient's stay in the hospital. We have contracts with all hospitals in our service area. What if a patient lives beyond six months? When the patient's physician signs the hospice certification, it signifies that he/she believes that, in their best medical estimate, life expectancy is six months or less if the illness takes its expected course. However, patients will not be discharged from our care if they exceed that time frame, as long as they meet the criteria for receiving hospice care. Does your organization care for children? VIA Health Partners is the only provider of a specialized children's program in our eight-county service area. The Kids Path® children's program specializes in caring for the unique needs of seriously ill children and their loved ones. What grief programs do you have for children? We offer individual sessions for children, grief camps for ages 7-16, and short term grief support groups in partnership with schools. What kind of community education is provided? Our specially trained staff and volunteers give presentations to civic groups, long term care communities, businesses, communities of faith, and other groups who want to learn more about our services and programs.
