Referrals may also be made by patients or family members. Individuals live in assisted living residences or skilled nursing facilities and require end-of-life care Community Education VIA Health Partners is proud to offer our community a wide range of services and training. From end-of-life planning to grief topics, we continually provide educational opportunities to individuals and organizations within our service area. Professional Education We offer many educational opportunities free of charge to the communities we serve. Many of our presentations have been approved for continuing education units for CNAs by the N.C. Division of Facility Services. These presentations also provide social work and nurse contact hours. Community Education VIA Health Partners remains highly visible through an array of community outreach efforts. Reimbursement for Services Covering Care Hospice care and palliative medicine services are routinely covered by Medicare and Medicaid. These services are also covered by most employer-provided health care plans, although the specific coverage will vary by plan. VIA Health Partners accept Medicare, Medicaid, and private insurance. Please check with your health care plan administrator to learn more about your own coverage for hospice care and palliative medicine services. VIA Health Partners believe that a patient’s financial situation should not be a barrier to receiving quality healthcare, so we provide need-based financial assistance that is funded through community support. If you have private insurance, VIA Health Partners will contact your insurance company prior to admission to determine your policy’s specific benefits. Since 2013, Mike has served as HPCCR’s VP & Chief Operating Officer. A graduate of Duquesne University, Mike earned his Doctor of Pharmacy in 2002. Subsequently, he earned his MBA from Pfeiffer University and is also a Certified Hospice and Palliative Care Administrator (CHPCA). Mike has served as Board Chair for NCMedAssist and is a board member of the Association For Home & Hospice Care of NC. He also serves on the Pharmacy Steering Committee of the National Hospice and Palliative Care Organization. Caring for a loved one with dementia is difficult, but you're not alone... Caring for a loved one with dementia is often very difficult, with family and other caregivers experiencing high levels of both physical and emotional stress. In an effort to alleviate some of this anxiety, VIA Health Partners has created a specialized care plan for patients diagnosed with dementia, which also includes support and education for caregivers. The extensive training provides tools to physicians, RNs, nursing assistants, social workers, chaplains, and volunteers and includes instruction on increasing or maintaining a patient’s quality of life, appropriate pain and symptom management, and Advance Care Planning (i.e. Living Will, Health Care Power of Attorney). There is even social support for caregivers. We have trained clinical staff to understand the symptoms of dementia and we have introduced a pain assessment tool specifically designed for patients with impaired communication. These teams also work with therapeutic sensory kits containing aromatherapy scents and tactile stimuli, incorporating items reminiscent of their patients' childhood or career, music from their youth, and more. Our staff have had great success using iPads with specific applications to interest patients with dementia. Social workers use iPads with specific applications that have produced interaction with patients. Over the past several years, we have seen amazing results. Even our most unresponsive patients have interacted with the iPads that have been brought to visits. A diagnosis of dementia can be frightening for a family, but this new program offers a glimpse of the loved one they know is still there. This initiative is being implemented in homes, assisted-living communities, and skilled nursing facilities. We believe that this new program will make a significant impact in the end-of-life care that our patients with advanced dementia receive. In order to offer even better care to our patients living with dementia, VIA Health Partners has a goal of purchasing iPads for all of our social workers to use with this project. You can help us reach this goal by making a gift to fund this meaningful project. Thank you to the Mecklenburg Medical Alliance & Endowment for their grant to support "Positive Approach to Care," a hospice training program that provides volunteers and staff with a deeper understanding of dementia and dementia related behaviors.

How is Hospice paid for? Hospice Care is available to all, the ability to pay will never be a barrier... Hospice is an inclusive Medicare Part A benefit, meaning that all services related to terminal illness are covered up to 100%. Hospice care is also covered by Medicaid and most employer-provided healthcare plans. However, at VIA Health Partners, ability to pay will not be a barrier to receiving needed care. Thanks to generous community support, and our non-profit mission, our services are available to anyone in our community who needs us. For individuals enrolled in Medicare, hospice is a benefit that they are entitled to have. The Hospice Medicare Benefit completely covers the cost of all hospice services, medical equipment, supplies, and medications related to the hospice diagnosis as long as these items are included in the hospice plan of care.
