We support. We care... Grief comes at different points along the journey of loss, but the primary points are the period that begins at diagnosis and the period that begins at the death of a loved one. From the point of diagnosis to death, there are two types of grief: preparatory and anticipatory . Preparatory Grief is experienced by the person who is ill as they grieve the loss of their health, independence, and the shortened time they will have with others. Anticipatory Grief is experienced by family and friends as they struggle with thoughts and feelings surrounding the impending loss of their loved one. In the hospice and palliative care settings, these losses are typically processed with social workers and/or chaplains who are specifically trained to deal with these experiences. Once death has occurred, feelings, thoughts, physical reactions, and other aspects of grief can make adjustment difficult, and carrying out basic activities may seem harder. Family, friends, and other people you know and trust may have difficulty sharing in your grief or they may be struggling with the loss themselves. That’s when the grief care services of VIA Health Partners can be so important. We listen. We support. We care. Our staff of professional counselors has a wide range of experience in grief counseling. All hold professional licenses and/or certifications and have degrees covering social work, counseling, and divinity. Counseling for adults, children and teens is available throughout our 49-county service area. Our grief counselors create a safe environment for individuals who are adjusting to a significant loss. If you have survived the loss of a loved one, we can help you discover, or rediscover, inner resources that will promote healing and help you define a meaningful future. Individual counseling. . . Talking one-on-one with a grief counselor helps to make sense of the grief process. The way in which a person grieves is unique to that person. Having a grief counselor to help you work through your emotions and thoughts about your loved one, share your memories, and discuss adjustments to life is healthy; it helps you recognize that what you are going through, though painful, is a natural life experience. The support group allows group members a safe environment to experience empathetic support and a place where social isolation and feelings of loneliness are diminished. Groups specific to spousal/partner loss, adult parent loss, adult child loss, young widow/widower, mature widower loss, and child loss may also be offered. A pre-group interview with a grief counselor is required for members of structured grief groups. Please contact us for more information. "In Good Company" Monthly Drop-ins If you are interested in a more relaxed, informal group setting, come to one of our "In Good Company" support groups . We offer camaraderie and an opportunity to check in with a grief counselor (and others) to share the highs and lows common to the experience of people who are grieving. In School Grief Groups School personnel are encouraged to contact us with requests for in-school grief support groups. In other words, we not only care for the family members of patients under our care, but we will offer our services free of charge to anyone who is struggling with loss. Any individual in our service area may sign up for our grief support groups or workshops, or contact a counselor for additional assistance. Memorial Services Remembering those we love and have passed away is an important part of individual and family identity. Attending a memorial service is healing, connects the present with the past, and continues the bond of a loving relationship. Each year, family survivors and community members receive invitations for our Light Up a Life interfaith memorial services held throughout our 49-county service area. An advance directive is a legal document that outlines choices about being kept alive by artificial means when suffering from a terminal illness, permanent brain damage, or advanced dementia. Such documents are utilized when we lose decision-making capacity. While many of us have strong opinions about whether we would like to receive these types of treatments in an attempt to keep us alive, few of us actually take the steps to ensure that our wishes are honored. Studies have found that most of us would refuse to be kept alive by artificial means if we have dementia or a terminal illness*. Studies have also found that only 50% of terminally ill patients execute advance directives**. However, there are several easily completed documents that will allow loved ones to understand a patient's wishes regarding end-of-life care, even if the patient lacks decision-making capacity. The state of North Carolina recognizes two advance directive documents. Revised in 2007, they are the Heath Care Power of Attorney and the Living Will. These documents are legal when they are completed by an individual with decision-making capacity, signed by two witnesses, and notarized. There are also two medical orders recognized by North Carolina. They are the Do Not Resuscitate (DNR) and Medical Orders for Scope of Treatment (MOST) forms. These forms are obtained from the physician or healthcare provider. Health Care Power of Attorney Health Care Power of Attorney The Health Care Power of Attorney allows a person to name someone to make healthcare decisions for them, should the person be unable to communicate their wishes. Advance Directive for a Natural Death (Living Will) Advance Directive for a Natural Death (Living Will) The Living Will should be used as instructions for the future for anyone who would like their healthcare providers to withhold or withdraw life-prolonging measures in certain situations. There is no legal requirement that anyone execute a living will. Do Not Resuscitate (DNR) Do Not Resuscitate (DNR) The DNR form is a medical order that communicates a person’s desire to forgo cardiopulmonary resuscitation should their heart stop beating and they stop breathing. Caring for a loved one with dementia is difficult, but you're not alone... Caring for a loved one with dementia is often very difficult, with family and other caregivers experiencing high levels of both physical and emotional stress. In an effort to alleviate some of this anxiety, VIA Health Partners has created a specialized care plan for patients diagnosed with dementia, which also includes support and education for caregivers. The extensive training provides tools to physicians, RNs, nursing assistants, social workers, chaplains, and volunteers and includes instruction on increasing or maintaining a patient’s quality of life, appropriate pain and symptom management, and Advance Care Planning (i.e. Living Will, Health Care Power of Attorney). There is even social support for caregivers. We have trained clinical staff to understand the symptoms of dementia and we have introduced a pain assessment tool specifically designed for patients with impaired communication. These teams also work with therapeutic sensory kits containing aromatherapy scents and tactile stimuli, incorporating items reminiscent of their patients' childhood or career, music from their youth, and more. Our staff have had great success using iPads with specific applications to interest patients with dementia. Social workers use iPads with specific applications that have produced interaction with patients. Over the past several years, we have seen amazing results. Even our most unresponsive patients have interacted with the iPads that have been brought to visits. A diagnosis of dementia can be frightening for a family, but this new program offers a glimpse of the loved one they know is still there. This initiative is being implemented in homes, assisted-living communities, and skilled nursing facilities. We believe that this new program will make a significant impact in the end-of-life care that our patients with advanced dementia receive. In order to offer even better care to our patients living with dementia, VIA Health Partners has a goal of purchasing iPads for all of our social workers to use with this project. You can help us reach this goal by making a gift to fund this meaningful project.     Dr. Bridget Hiller, Chief Medical Officer, VIA Health Partners Chief Medical Officer Dr. Bridget Hiller, MD HMDC About Dr. Hiller received her medical degree from State University of New York Brooklyn. She completed her residency in Internal Medicine at North Shore University Hospital 2005 and went on to serve as Chief Resident. She completed her fellowship in Hospice and Palliative Medicine in 2012, became fellow for the Academy of Hospice and Palliative Medicine in 2019, and also passed her Hospice Medical Director certification in 2019. In 2021 she was chosen as one of 50 women physicians as a Woman’s Wellness through Equity and Leadership (WEL) Scholar from the American Hospital Association. Dr. Hiller’s experience and background in improving access to care for patients living with serious and life-threatening illnesses will be of vital importance as we continue to provide excellence in care to the communities we serve.
