CHARLOTTE, N.C., September 2, 2025 — VIA Health Partners has expanded its range of services by welcoming Equity Health, a Denver, N.C.–based provider of home-based primary care and wellness services, into its organization. The partnership strengthens VIA’s ability to deliver personalized, coordinated care, especially for patients living with serious illnesses who benefit from being treated at home.
Equity Health is recognized for providing high-quality medical care where patients live, reducing the stress of travel, removing barriers to treatment, and ensuring continuity of care.
